Discharge Administrator
Transfer of Care Hub (TOCH)
Grade: 5
Hours (per week): 36 hours per week
Responsible for:
The Core Discharge Team administrator will provide comprehensive administrative support to one of three teams of Discharge Facilitators and Discharge Navigators, working collaboratively with the other two Administrators to provide a seamless service.
The Transfer of Care Bureau (TOCB) is a co-located, hospital-based team consisting of the relevant services (acute, community, primary, social care, and specialist services including housing, voluntary sector, etc) to coordinate the discharge of Croydon residents from Croydon University Hospital into the appropriate discharge setting.
The cornerstone of this team is the ‘core discharge team’, a ward-based team with a wide range of skillsets and backgrounds including nursing, therapy, and social work. The team will co-ordinate all aspects of supported discharges and as the ‘single point of discharge’ from CUH. The team includes roles where traditional professional boundaries and responsibilities have been blurred, to streamline discharge processes and improve experiences for patients and staff.
The administrator will support communication between the IDT and relatives, carers, ward teams, the wider TOCB and other external organisations to promote an effective and smooth discharge for all patients on their designated wards.
A crucial element of the post holder’s role will be ensuring accurate data is captured on all data bases to fulfil all Trust and Council reporting requirements. The administrators will need to read and record information on both CHS and Croydon Council information systems, to ensure that up to date information about the factors affecting a patient’s discharge are visible to all members of the MDT.
The post holder will be required to work on site and be flexible to meet the needs of the service. This may include some weekend work.
