Team Manager – LIFE Service (Discharge to Assess Pathway)
Location: Bernard Weatherill House - 8 Mint Walk, Croydon
Grade: 15
We are seeking a dynamic and experienced Team Manager to lead our LIFE Service – Discharge to Assess (D2A) pathway. This is a pivotal role focused on prevention, early intervention, and whole-family support, helping Croydon residents live independent, fulfilling lives.
Key Responsibilities
- Lead and manage a multidisciplinary team of Social Workers, Therapists and Health & Wellbeing Assessors.
- Oversee the operational delivery of the D2A pathway, ensuring high-quality, person-centred care.
- Use tools like the Community Led Support resource wheel to support strengths-based conversations and planning.
- Foster strong partnerships across health, social care, and the voluntary sector.
- Ensure compliance with statutory guidance and contribute to the transformation of health and social care services.
- Promote a culture of continuous improvement, innovation, and positive outcomes for residents.
You will be a confident and compassionate leader with:
- A recognised social work qualification and current registration with Social Work England.
- Proven experience in adult social care, ideally within hospital discharge or reablement services.
- Strong understanding of strengths-based and community-led support models.
- Excellent communication and partnership-building skills.
- A commitment to embedding cultural and behavioural change that puts residents at the heart of everything we do.
